Paul Mendoza: Licensed Massage Therapist Dress Code: Every Attempt is made to provide a modest and safe environment for the client as well as the therapist. Clients may partially undress to their underwear; however they feel the most comfortable. Clients will be modestly draped and covered at all times. Only the area being massaged will be undraped. Personal Disclosure: Clients will be freshly bathed prior to the massage appointment. There will be times when general discussions take place during the massage session. However, as a therapist, it is my focus and intent to give you all my energies to enhance your experience. Therefore, concentration, focus and centering are required on my part. If you are interested in the modality I am using, please let me know and I will be happy to explain what exactly I am doing. Contraindications To Massage: Please inform the therapist if you currently have any of the following: bruised areas, open wounds, break in skin, or recent burns. Should the Massage therapist or client contract a contagious disease that could spread during the massage session (cold flu, other droplet or airborne infections), each will inform the other. The decision to reschedule will be handled in a case by case fashion. Tenderness in areas where direct pressure methods are used in a possible reaction for up to 48 hours. If you are sore for longer than two days, the massage needs to be adjusted to be gentler. Please let me know if this occurs. Persons with uncontrolled high blood pressure, heart disease, recent surgery or fractures, phlebitis, fever should consult their physicians before receiving a massage, and will need a written consent from the physician before the massage can take place. Inform the therapist of any restrictions that may apply. Ethics: Everything communicated is held completely confidential. All personal client information is confidential. Client's records are available for their review upon request. However, the massage therapist may be required to release a client's records to the client's doctor with client's permission or under court order. Anyone under the age of 18 will not be seen without a parent or guardian present in the therapy room. The therapy room door is never locked Informed consent agreement: Massage is a form of bodywork that dates back more then 3,000 years. It involves the manipulation of tissues through touch to promote a client's well- being and health. The benefits of Massage therapy include improved circulation, digestion, immunity, flexibility, breathing, posture, and general health; reduction of stress and anxiety; increase clarity; increased energy; and greater sense of the mind-body connection. I am a graduate of The Arizona School of Massage therapy. Prices for a Massage: 50 minutes- 50$ 90 minutes- 65$ Work hours: I work everyday from 8 am to 6pm. Excluding holidays. Payment for the full missed session will be required. If you are late for your appointment, please understand that you may not be able to receive a full session. Sexual behavior by the client towards the therapist is always unethical and inappropriate. Such behavior will result in immediate termination of the session with payment due in full. Informed consent: It is the client's responsibility to keep the massage therapist informed of any current medical treatment and to provide requested written permission from physician, chiropractor, physical therapist, or other health care provider to the massage therapist so that the massage may be continued. Massage therapists do not diagnose or prescribe for a particular body function, but may choose to do to prolong the effects of massage and maintain a healthy lifestyle.
