The Right Team, LLC is the provider of individualized nursing care management. We begin with a detailed multidisciplinary assessment. This is the guide we use to form a comprehensive nursing care plan to include spiritual, emotional, and physical needs, and the planned action to meet these needs. We provide care in the client's home of choice, private residence, shared housing, continuing care facility, assisted living facility, dementia care unit, acute care rehabilitation unit, and other long-term care settings. We always meet the client at their present functional level, but strive for the least restrictive, safe environment when assisting with placement.
Our nurses provide hands-on assessments on regular home visits, including medication set-up, vital sign evaluation, and medication review. We utilize an extensive list of competent professional agencies for referrals such as physical and occupational therapy, speech therapy, personal training, music therapy, pet therapy, home care providers, and specialized services of geriatric physicians and neuro-psychological medical personnel.
Through evaluation, implementation of care plans, and ongoing assessments, we provide an outstanding level of care to our seniors.
Our company is not part of a franchise, so policies and procedures are nursing-based, and all referrals are monitored by The Right Team, LLC.

Donna Chmielewski developed the care plan, helped identify a care provider, provided medical check-ups and monitored service delivery for my mother who has Alzheimer's over the period of a year. I highly recommend Donna Chmielewski and the Right Team to anyone looking for a geriatric care manager. I have employed Donna since about July 2005 to assist with evaluating and monitoring the health care needs of my parents. She has been invaluable in providing advice and guidance to my father about his own health needs and his efforts to care for my late mother as she increasingly suffered from dementia. Donna did a tremendous job in overseeing my elderly Mom's care, while she resided in a nursing home. She attended care conferences, doctors visits, with me, and in my absence. At every nursing home visit she consulted with nurses and other staff in regard to Mom's needs. She kept me completely informed after every visit she made to the nursing home. But most importantly she offered my Mom such compassionate care and friendship. My Mom loved her. Donna was invaluable to me in so many ways in helping with my Mom's care. I always referred to her as our "Angel". She was wonderful in every respect.

Donna was my 98 yr. old Aunt's personal care nurse and took a very personal interest in her well being.
She was always there when needed and made regular visits to make sure that she was doing well and took care of her medical needs and Doctors visits.
Since I was my Aunt's Legal guardian she kept me informed almost daily.

A. Home care agencies advertise that they provide care managers. Be aware that not all care managers are the same in education or skill. A nurse care manager has the education to critically evaluate the status of an patient/elder.
Much more is involved than what meets the eye.
A nurse is a professional who is licensed in the state in which she/he practices. There are a specific set of skills required by the state both educationally and practically, before an examination is given. Ongoing continuing education is required in most states to continue to hold an active license.
A. How does your first interaction with the care manager feel?
Does the care manager answer your questions?
Give the care manager a situation and then ask her/him to explain what her approach to solving the problem would be.
If you feel comfortable with her response then, you have your answer.

Does your parent have a power of attorney for healthcare and property?
Have you discussed your concerns with your elder?
What is your elder's response to your concerns?

A. How do you convince a client that she/he need more care at home?
My first step is asking them about themselves. The next step is listening to what they say and more importantly, what they don't say. Reading the nonverbal clues can often help the care manager address those things that are really concerning the senior which are often:
"Will I be able to stay at home? How can I trust someone helping me? Are you trying to put me in a nursing home?"
I tell them we are going to help them stay safe and try to avoid any crisis which might change their ability to remain at home.
A. Nursing is a vocation. The best thing about this profession is that it requires you to use many different skills. It requires a good ear, insight, and patience, but it is as rewarding as it is challenging.
A. Continuing education is required to remain a licensed nursing professional in Illinois. We attend programs in gerontology, diabetes, cardiovascular disease, immune system diseases, dementia, and other areas of nursing and psycho-social issues relating to seniors and their families.
We attend conferences, seminars, and subscribe to many professional journals.
Both care managers have recently obtained certifications as Advanced Legal Nurse Consultants and Certified Dementia Care Professionals.
