We're an in-home service provider who assists individuals with disabilities, families and seniors. We're committed to helping our clients lead dignified, independent lifestyles in the safety and comfort of their own homes by assessing carefully and understanding their needs fully and by selectively placing experienced personnel to satisfy these needs.

We deliver a unique community-based service that will assist our clients and allow them to continue being independent and safe in their own homes. Our caregivers are carefully screened, supervised, trained, bonded, insured, and chosen on the basis of their commitment and their experience to caring for people at home.

Our caregivers are courteous in nature, caring, eager to help, and committed to care. Our screening process is based on only hiring those who we would have in our own home and would care for our parents.

We will treat you like a family member. We're not a big national franchise, but a small privately owned company that centers on our clients' well-being. You deal with real people, not a corporation.

We're passionate about the services we provide and are proud about our responsiveness and our capability to provide our clients with personal attention. County Home Care's services are flexible and can be arranged seven days a week, for as many as twenty-four hours a day or as short as a few hours a week. That includes holidays.

County Home caregivers can help with a wide range of activities including the following:

* Home Services

• Routine housework

- Changing beds
- Cleaning ovens and stoves and china cabinets
- Dusting
- General cleaning
- Ironing
- Defrosting refrigerators and freezers
- Vacuuming
- Washing floors
- Laundry

• Heavier cleaning such as washing walls and windows, spring cleaning and light fixtures
• Baking for holidays and special occasions
• Planning, cooking, and serving meals
• Transporting clients to social activities and appointments
• Motivating and monitoring regular diet plans
• Providing respite and palliative care
• Shopping for clients, running errands or transporting clients to carry out these activities
• Assisting with payment of bills, mail and correspondence
• Serving as companions and participating in friendly socialization activities
• Packing household goods when changing residences
• Assisting with entertainment preparations, hosting, serving and/or cleanup
• Caring for pets

- Cleaning litter boxes
- Cleaning birdcages
- Feeding
- Exercising
- Taking to grooming appointments or to the vet

• Caring for plants
• Checking interior conditions of homes while the residents are away and making sure the exteriors reflect an "at-home" appearance
• Monitoring clients' safety, comfort and welfare by making home visit(s) or contacting them by phone

• Assisting with grooming needs

- Cleaning teeth
- Hair care
- Routine nail care
- Supervising or assisting with showers, sponge baths, bed baths and tub baths
- Shaving

• Assisting with undressing and dressing
• Providing routine skin care (such as applying lotion to dry skin)
• Assisting with self-administered medications by reminding client to take them at the prescribed time(s) and/or following up to ensure they are taken
• Assisting with eating or feeding
• Assisting with mobilization activities including exercising, turning, walking and transferring
• Assisting with toilet and/or commode needs
• Providing respite and palliative care
• Monitoring vital signs, such as pulse, respiration and blood pressures

* Companionship

• Reading
• Engaging in conversation and discussing current events
• Rent and play movies and games
• Reminders for appointments
• Visiting friends and neighbors and running errands
• Transportation to social events, religious services or appointments

* In Home Assessments (Optional)

Non-medical assessments are administered to find clients' functioning capabilities to determine their needs and to find out what services are required to assist them attain and maintain optimum independence.

Who can obtain service?

Certain segments of the population will find our services highly beneficial, such as people who are:

When is service available? For seven days a week, twenty-four hours a day, help assistance is available to conform to clients' needs and preferences.

• Rockland
• Ulster
• Westchester

If a need arises, we will strive to accommodate anyone who requires service outside this geographical area.

• We follow up with our clients regularly to check effectiveness of service and to make modifications that reflect changing wishes and needs

How costs are determined:

We will be charging a variable rate depending on the kind of service needed, where it was delivered and when it was provided. There are no costs for non-medical assessments. Prior to commencement of service, costs are worked out and discussed with clients.

How to arrange for service

• Office Hours

Office hours are Monday to Saturday, from 8:00 a.m. to 6:00 p.m. Voice mail is available 24 hours a day and regularly checked, so that we can provide immediate attention to requests for prompt assistance.
