- What is your typical process for working with a new customer?
What to Expect As a creditor under the Federal Trade Commission’s “Red Flags Rule” we adhere to an Identity Theft Prevention Program. For this reason, we ask that you be able to present photo identification at the time of registration. We appreciate your cooperation in assisting us with protecting your identity and the prevention of identity theft. Acceptable forms of photo identification include: •Valid State Identification Card •Valid State Drivers License •School Identification Card •Valid Passport Please bring with you your Medical Insurance Card if applicable. We ask you to arrive at least 15 minutes prior to your scheduled appointment time in order to provide sufficient time to complete all necessary paperwork. We are pleased to offer translator services at no cost to our patients. If such a translator is not available, when scheduling the appointment or for any appointments, we ask that you obtain the services of your own translator. Please wear comfortable clothing. Please plan on staying for at least 1 hour or more for your initial assessment.
