- What should the customer know about your pricing (e.g., discounts, fees)?
I accept Medicare with their secondary insurances. We will verify your insurance before getting started to ensure there is enough coverage for your skilled outpatient physical therapy home visits. **Currently in process of credentialing with other insurance carriers** I also offer Cash-Based Telehealth (video face to face) physical therapy visits $100/hour with a Doctor of Physical Therapy. In-home cash-based services at $160 Initial PT evaluation and $150 for follow ups thereafter with a doctor of physical therapy. Patients also have the option of Initial PT evaluation at $160 from myself, a Doctor of physical therapy and follow up visits with my physical therapist assistant at $100 per visit.
- What is your typical process for working with a new customer?
The first step would be verifying your Medicare insurance coverage for outpatient PT services. If paying cash we proceed to scheduling. Second, scheduling the appointment. Third, I email you 4 forms (Consent to Treatment form, 24hr cancel form, medical history & medication form). Forms are to be emailed back to me prior to your Initial Evaluation. This saves some time during your one hour appointment. The evaluation consists of a thorough physical exam, identifying your deficits/weaknesses, sources/causes of pain, advising/explaining your plan of care and giving you treatment on the first day.

I make continuing education a priority. There is no way I could list all of them, but I’ve selected a few to share with you below: 2018, current- Certified in C.P.
