Introduction: Hello, I'm Angela, a seasoned Registered Nurse (Licensed in the state of California) of 27 years. My clinical background includes medical -surgical, orthopedics, neurology, hospice, geriatrics, psychiatry, telemetry, home care, cardiology, GI, occupational health, care management, out-pt case management, pediatrics, and dermatology. As a Virtual RN Health & Wellness Coach I educate and empower people to take an active role in improving their health and bridging the gap between you and your many providers. I understand the needs of people living with chronic conditions such as diabetes, asthma, high blood pressure, heart conditions, COPD, long COVID and more. I can provide the extra help to follow through with your provider’s instructions. I can provide personal attention in helping you meet your health goals. I can help you: - Coordinate care AND referrals with primary doctor, and specialists - Navigate your health plan (be your advocate for your healthcare benefits) - Understand your diagnosis and things you can do to manage it - Support you in making and showing up to medical appointments -Follow through on your medication or other prescription instructions - Link you to community resources, such as support groups - Discuss possible transportation choices to help you get to appointments -Develop an individualized plan to help you meet your health goals. I help clients explore what their ideal health looks like, and help them find the motivation and tools they need to make sustainable lifestyle changes. I emphasize goal setting and help clients create realistic, actionable plans to change the trajectory of their health.
