- What should the customer know about your pricing (e.g., discounts, fees)?
No Insurance? Don't worry - our Private Pay prices (for those without insurance) are: Initial PT Evaluation: $135.00 (about 1 - 1.5 hours) Each Follow-Up PT Treatment: $85 (about 1 - 1.5 hours) If you require more than 2-3 visits per week, we are happy to discuss a plan that will fit your budget and still keep you on track for your treatments. We have families, bills, emergencies, and the occasional unplanned medical event too -- We never want a financial burden that keeps you from getting the care you need, when you need it. If you have health insurance... please remember to bring the Script for Physical Therapy from your Physician (if you have it), and your insurance card with you to your Initial Evaluation. We're looking forward to seeing you soon! We gladly accept Worker's Comp, Medicare, PPOs, and most HMOs. If you have any questions or require more details in regards to your insurance benefits, please call us! We are happy to verify your benefits prior to your first visit.
- What is your typical process for working with a new customer?
Required paperwork is the pits, and we know it. So we've simplified it as much as possible. You can download your Initial Evaluation Forms here (in both English and Español) - Five pages total are included: the Registration Form, the Medical Records Release, and the Past Medical/Surgical History Forms. These forms are available under "Forms" on our website. If you have completed this paperwork prior to your first visit, please bring it with you, and plan to arrive 5-10 minutes early with your PT Script and your Insurance Card(s). If you were not able to complete this paperwork prior to your first visit, please plan to arrive 20-25 minutes early for 'Fun Paperwork Time' - and don't forget your PT Script and your Insurance Card(s).
- What education and/or training do you have that relates to your work?
Dawn Marie received her Bachelors in Marketing and a Minor in Music Percussion from Oklahoma State University, attended The University of Kansas for her PT prerequisites, and graduated with her Masters in Physical Therapy in 2006 from Rockhurst University in Kansas City, Missouri; she later received her Doctorate of Physical Therapy degree from Texas Tech University. Dawn specializes in Orthopedic, Worker's Comp, and Sports-related injuries, is Certified in Applied Functional Sciences, and is working towards her Certification in McKenzie Mechanical Diagnosis & Therapy.
