I and my husband have been struggling with our weight for over 2 years now after having kids together. Health & personal training documents required to be completed a. To examine clients medical history & health limitations. b. To inform client and agree to of personal training stipulations and terms. 3. Facilitation of training schedule - To organize days & times of training.
