In some cases you may be asked to privide a second form of ID, i.e. signed social security card in order to match the signatures. Take in to account that a Notary cannot prepare any document for you or advise what type of Notary act is needed, you need to tell the Notary what it is you need and do not sign the document until you are infront on the Notary.
